Must meet one of the following criteria:

  • The client is in receipt or eligible to receive other long term care community services
  • The client is isolated and requires social interaction or stimulation

Transportation:

  • Unable to access public transportation because of short or long term physical limitations
  • Requires escort during the trip because of frailty or cognitive impairment
  • Specialized transit is not available or the individual does not meet the eligibility for Handi-Transit
  • Public transportation is not available and friends or relatives are not available to provide transportation

  • Requires completion of a photo identification card (valid for 1 year); done during initial visit

  • Must not be able to access the public transportation services
  • Must not be able to drive themselves
  • Must not live in a long term care facility
  • Must be able to get in and out of the vehicle with minimal assistance

Wheelchair Passengers:

  • Wheelchair bound passengers must be accompanied by companion and he/she is required to provide all assistance
  • Companions assisting passenger receive transportation for free but all other charges including accommodations, are at their own expense
